ALL PRESCRIPTION PAD ORDERS WILL SHIP DIRECTLY TO THE ADDRESS ON FILE WITH THE STATE OF NEW JERSEY.
You are responsible for making sure it is shipping to right address, and APTCO will not be held liable for shipping to wrong address.
IF YOU WISH TO SHIP TO ANOTHER LOCATION, YOU MUST CHANGE YOUR ADDRESS WITH THE STATE OF NEW JERSEY AND SEND US THE APPROVAL FORM FROM THE STATE OF NEW JERSEY with the new address attached.

- If you have a copy of your last script, please send with form
- Please remember to add SUITE/BLDG or FLOOR if applicable in address line. Shipping companies are now charging a redelivery fee, and it will be added to the freight.
